laser photocoagulation in diabetic macular edema: effects on visual acuity and macular edema

چکیده
due to the importance of clinically significant macular edema in diabetic patients, this study is aimed to determine if laser photocoagulation is effective in the treatment of clinically significant diabetic macular edema. in addition, the effects of risk factors arc surveyed* this is an existing data study considering patients with clinically significant diabetic macular edema, treated with argon-green laser photocoagulation in labbafinejad hospital, department of lasertherapy, from 1995 to 1997. in 60 (42.6%) eyes the treatment method was focal, in 22 (15.6%) eyes grid, and in 59 (41.84) modified grid laser photocoagulation was performed. the results are based upon deterioration of visual acuity, occurance of moderate visual loss and improvement or persistence of csme. we studied 114 eyes from 87 patients. two years after initial treatment, visual acuity improved in 19.1% of eyes, unchanged in 9.5% and worsened in 71.4% of eyes. after this period the rate of moderate visual loss was 28.6% and csme was improved in 23.8% of eyes. according to our study, baseline visual acuity and retinopathy severity were two important intervening factors in response to lasertherapy. comparing our results with natural course of diabetic macular edema, indicates that in assessing visual outcome laser photocoagulation is an effective modality in treatment of csme, but it is not effective in maintaining or improving visual acuity, which is due to patients delay in visiting ophthalmologists and paying not enough attention to follow-up visits.

Correlation between Visual Acuity and Optical Coherence Tomography-Measured Retinal Thickness in Diabetic Macular Edema
Background and Objective: Diabetic macular edema (DME) is one of the common complications of diabetes which significantly accounts for preventable visual impairment and blindness. Central macular thickness (CMT) is a feature found in DME patients. This study aims to determine the relationship between optical coherence tomography (OCT)-measured CMT and visual acuity (VA) in patients with DME bef...
متن کاملIs Laser Photocoagulation Treatment Currently Useful in Diabetic Macular Edema?
Diabetic macular edema treatments have changed in recent years after the introduction of intravitreal injections of anti-vascular endothelial growth factor (VEGF) (ranibizumab, bevacizumab, pegaptanib and the most-recent introduction of aflibercept), corticoids (triamcinolone or dexamethasone), and the sustained delivery fluocinolone acetonide vitreous inserts. All these new drugs are becoming ...
متن کاملRemote Ischemic Preconditioning and Diabetic Macular Edema
Objective: Remote Ischemic Preconditioning (RIPC) as the transient ischemia and reperfusion of the arm is a promising method for protecting different tissue from future ischemia. These effects might be mediated through vascular and endothelial growth factor (VEGF) pathway. We investigated the influence of RIPC on diabetic macular edema (DME) as a chronic ischemic condition in patients who were ...
متن کاملFocal Laser Photocoagulation in Non-Center Involved Diabetic Macular Edema
This study was performed to evaluate the functional and anatomic outcomes of focal macular laser photocoagulation in eyes with non-center involved macular edema (non-CI ME). Forty-nine eyes of 43 patients with non-CI ME were included. Focal macular laser photocoagulation was conducted on twenty-nine eyes of 25 patients, while 20 eyes of 18 patients with non-CI ME were followed without treatment...
متن کاملInfliximab for Diabetic Macular Edema Refractory to Laser Photocoagulation
OBJECTIVE Because many patients with diabetic macular edema (DME) do not respond to focal/grid laser photocoagulation, the only currently approved treatment, alternatives are needed. Based on encouraging preliminary findings, we aimed to assess efficacy and safety of the anti-tumor necrosis factor (TNF) monoclonal antibody infliximab in this condition. RESEARCH DESIGN AND METHODS This was a s...
